Wisdom Tooth Removal

Wisdom teeth only need removing when they are causing problems, or are clearly going to. A wisdom tooth that has come through fully, is cleanable and is not causing trouble is usually best left alone.

They do need to come out when they are partly through and repeatedly getting infected, when they are decaying the tooth in front, or when there is not enough room and the pressure is causing pain. We assess rather than remove routinely.

Symptoms & Indications

You might need this if…

  • Recurring pain or swelling at the back of the jaw
  • Gum around a partly erupted wisdom tooth that keeps becoming infected
  • Difficulty opening the mouth fully, or pain on swallowing
  • Food constantly packing behind the last tooth
  • Decay in the wisdom tooth or in the tooth in front of it
  • A tooth broken down too far to restore
Clinical Workflow

What happens, step by step

  1. 1

    Assessment

A radiograph shows the position of the roots and how close they run to the nerve in the lower jaw. For difficult positions we may ask for a CBCT scan, or refer you to an oral surgeon — where that is the safer route, we will say so.

  2. 2

    Before the day

We go through your medical history and any medication, particularly blood thinners. Eat beforehand, and arrange for someone to take you home if we have agreed on sedation.

  3. 3

    The extraction

Local anaesthetic. Straightforward teeth come out with careful controlled movement. Impacted ones may need the gum lifted, some bone removed, or the tooth sectioned. You will feel pressure but should not feel pain.

  4. 4

    Closing the site

Stitches where needed, and a gauze pack to bite on. We will not let you leave until the bleeding is controlled and you have written aftercare instructions.

  5. 5

    Review

A check at about a week to confirm healing and remove any non-dissolving stitches.

Common Questions

Frequently Asked Questions about Wisdom Tooth Removal

Does it have to hurt?

The extraction itself is done under local anaesthetic, so you feel pressure rather than pain. Afterwards there is soreness and swelling for a few days, controlled with the pain relief we prescribe.

How long is the recovery?

Most people are comfortable within three to five days for a straightforward extraction, and up to about a week for an impacted one. Swelling is usually worst on the second or third day, then improves.

Should I have all four out at once?

Not necessarily. It depends on their position, your symptoms and your own preference. Doing one side at a time means you can still chew on the other, which many people prefer.

Can I go to work the next day?

For a simple extraction, often yes. For an impacted lower wisdom tooth, plan a day or two of rest. Do not schedule it right before an important event or travel.

Aftercare & Recovery

Guidelines from Dr. Srushti Patel following your treatment:

  • Do not rinse, spit or use a straw for 24 hours — it dislodges the clot and causes a dry socket, which is genuinely painful.
  • No smoking for at least 72 hours. It is the main cause of dry socket.
  • Cold compress on the outside for the first day; expect swelling to peak at 48 to 72 hours.
  • From the second day, gentle warm salt water rinses after meals.
  • Call us if bleeding will not stop, swelling worsens after day three, or pain increases rather than decreases.
